Guide to Reporting on Car Seat Checks

Before the Event

  1. If you have not done so, register for a National Digital Check Form (NDCF) account at least 3 business days prior to your event at www.carseatcheckform.org
  2.  Download the mobile NSC Car Seat Check Form app from Apple, Kindle, or Google Play. 
  3.  You can also use the paper Check Form. Download the latest version at www.carseatcheckform.org
  4.  For more information on the NDCF: 

During the Event

  1. Fill out the OPRC Car Seat Eligibility Certification Form (Eligibility Form) for each car seat distributed.
  2. Record car seat check information using the mobile app (no internet required) or paper Check Form.

After the Event

  1. Submit the Check Form data.
    • Paper: Enter the information from the paper Check Form into the NDCF database.
    • App: Connect to the internet and click “submit” to upload the data to the NDCF database.
  2. The NDCF will generate a unique ID number for each Check Form. Record the number on the Eligibility Form.
  3. Within 30 days:
    • Go to your OPRC account at https://www.floridaoprc.ce.ufl.edu/.
    • Click “profile” and scroll down to your current event.
    • Click “report” and fill out the number of car seats distributed. If you have car seats remaining, create a future event to transfer the remaining car seats to the new event.
    • Attach the signed Eligibility Form(s) as a single pdf file.
    • Ensure that there are no special characters in the document file name. (@#$%^&*_+/=)
